Many children and teens face barriers to quality health care due to geographical, financial, or other challenges. Located in twelve elementary, middle, and high schools throughout the Bronx and Queens, Urban Health Plan’s school-based health program delivers essential services directly to students, breaking down barriers such as transportation, cost, and scheduling challenges.
Regular checkups and screenings are essential for maintaining good health and can detect and address health issues early, preventing more serious complications. By providing care on-site, we help students avoid missing school for medical appointments, improve attendance, boost academic performance, and support their overall well-being.
Urban Health Plan’s school-based health programs are available to all students regardless of a family’s ability to pay.

Students may be referred to any of Urban Health Plan’s health centers for specialty care or follow-up for services not currently provided at the school-based health center.
