Urban Health Plan has made quality a central mission of its service delivery. In 2005 we introduced the Institute for the Advancement of Community Health (IACH), serving as UHP’s internal Quality Improvement (QI) arm. The IACH oversees the agency’s Quality Management Plan, updated annually under the direction of the CEO, with clinical quality under the direction of the Chief Medical Officer (CMO).

With the overall mission of “improving the health status of underserved communities by developing and disseminating innovative best practices,” IACH ensures adherence to the evidence-based approach of the Care Model: that the highest quality care is delivered to patients in the most cost-effective manner, through objectively, systematically, and continuously evaluating and monitoring clinical services and operations.
