Children are important members of our community, and Urban Health Plan proudly supports them through school-based health centers like our location at P.S. 161 – Ponce de Leon Elementary. Students from Pre-K to Special Education can access a range of services, including vaccines, well-child care, psychological support, and social services.
We’re committed to helping every child grow up healthy and strong by promoting positive health practices through education. Our bilingual professionals provide compassionate, high-quality care, with walk-in services available to all students, regardless of a family’s ability to pay.
