Urban Health Plan is one of the largest federally qualified health center systems in New York State proudly serving communities in the Bronx, Corona, Queens, and Central Harlem. Founded in 1974 by Dr. Richard Izquierdo, a life-long community resident of the South Bronx who dedicated his life to social justice and achieving health equity for all, today, the organization serves more than 89,000 patients who make above 430,000 visits.

Since 1974, we have been proudly bringing high-quality healthcare to New York families and have received national recognition for our approaches to community health care and disease prevention.
Today, we’re just as committed. Our network of community-based, patient-centered health centers brings state-of-the-art technology and comprehensive, coordinated care to people from all walks of life – from infants to seniors and everybody in between. All our health care facilities offer native Spanish-speaking providers, plus several other languages to serve our diverse community. We also cater to the needs of the LBGTQIA+ community, offering impartial, equitable care to all who walk through our doors.
Our sites in the South Bronx serve the residents of Mott Haven, Morrisania, Hunts Point, Longwood, Third Avenue and Soundview areas. Our Plaza del Sol Family Health Center is in Corona, Queens, serving patients in the surrounding communities of Elmhurst, East Elmhurst, and Jackson Heights. And our CitiCares Community Health Center serves residents in Central Harlem and surrounding neighborhoods.
