- Founded in 1974 by Dr. Richard Izquierdo, a local family practitioner who has been practicing in the community since 1962, as a not-for-profit, Article 28 Diagnostic and Treatment Center by the New York State Department of Health
- Federal Qualified Health Center (FQHC) designation obtained in 1999
- Accredited by the Joint Commission in 2003; reaccredited in 2006 and 2010
- Named one of the top twenty community health centers in the country; received the Davies Award for Excellence from the Health Information Management Systems Society for its excellence in the use of electronic health records

Awarded Level 3 recognition as a Physician Practice Connections – Patient-Centered Medical Home (PPC-PCMH) from the National Committee for Quality Assurance (NCQA). Level 3 recognition is the highest awarded by the NCQA.
